The questionnaire provides additional information about your plat map.  If you need assistance in completing your questionnaire, or locating your plat map, give us a call at 317-823-1586.

When filling out the questionnaire, please do so with your spouse and other family members as appropriate.  The more input you get from those who will be sharing your outdoor living spaces the better.  Please also do not feel you need to agree on, or answer every question.  I will be using your responses as a springboard for discussion of your specific needs, and look forward to working with you towards a creative, cost-effective landscape attuned to your needs.
